Diversity Issue Confidential Reporting Form

Dear Students, Faculty, and Staff,

The Diversity Committee is committed to ensuring that VLGS is a safe and welcoming environment to students from all backgrounds; it seeks to listen to its underrepresented groups and ensure that our community, on and off campus, is one that is a welcoming place for all of its members. Part of this work includes collecting information from individuals who have had or continue to have experiences that leave them feeling unwelcome, misunderstood, or threatened.

Below is an opportunity for students, staff, and faculty to report any incidents or experiences that made you feel less than welcome in the community as it pertains to your race, ethnicity, sexual orientation, gender identity, socio-economic status, age, ability, or religious beliefs; this information will remain anonymous but will be addressed by the Associate Dean of Diversity, Equity, and Inclusion, Lisa Ryan. Depending on the information provided, the Associate Dean may follow up with the person(s) responsible for your experience to gather additional information and inform them of your experience.

Any sexual assault will be reported to Professor Durkis-Stokes as a Title IX incident and, as such, will need to be reported appropriately. If this is a report of a sexual assault incident, please go to studentaffairsanddiversity to fill out the confidential reporting form.
